The period following discharge is a critical period for psychiatric patients. Psychiatric outpatient services were put in place to provide community based follow up for discharged patients and significantly determines the medical outcome of the patient and the probability of readmission.
Today, large numbers of psychiatric patients lack the much care needed after discharge which leads to increased numbers of readmission. According to research, thirty-seven percent of psychiatric patients are readmitted to hospitals within one year of their discharge (Agency for health care research and quality). This is attributed to many factors including inadequate discharge planning, lack of adhering to medications, patients lacking support from their families, patients are unable to take care of themselves among others. Furthermore, most psychiatric patients do not adhere to follow-up schedules after discharge.
